• Opens the Call
• Protects and secures members Protected Health Information (PHI).
• Reviews HRA and completes Mode of Care
• Based on identified needs, creates an ICP (Identified Care Plan).
• Identify AOF (Area of Focus) and come up with active interventions based on AOF.
• Completes health teaching.
• Documents in the client system.
• Closes the call

Education: Bachelors degree in allied sciences, Social Sciences, Social Work, Human Services, or a related field.

Experience:

• Minimum of 1-year experience in Direct Patient Care, such as in Long-Term Care, Home and Community-Based Services, Hospital/Facility Settings, Or Case Management.
• Experience working with managed care organizations, including Medicare and/or Medicaid recipients, preferred.
• Familiarity with BPO healthcare (member) processes, ideally in a Philippine setting.
• Note: Candidates with only BPO healthcare experience- handling member calls for a year or more in direct patient care will also be considered.

Technical Skills:

• Intermediate to advanced proficiency in Microsoft Word, Excel, and Outlook.
• Ability to use various electronic information applications and software programs, including electronic medical records (EMRs).
