Registered Social Worker

• Conduct comprehensive assessments to identify the needs and challenges of individuals and families in the community.
• Develop and implement personalized care plans in collaboration with clients, helping them set achievable goals and overseeing their progress.
• Provide counseling, emotional support, and crisis intervention to individuals and families in need.
• Connect clients to available community resources, such as healthcare services, housing options, employment assistance, and financial support programs.
• Advocate for clients' rights and coordinate with other professionals, agencies, and organizations to ensure they receive appropriate services and support.
• Facilitate support groups and educational workshops to enhance clients' social and life skills.
• Conduct home visits and follow-up appointments to monitor clients' well-being and address emerging needs.

  Bachelor's in Social Work, Registered Social Worker

 Strong understanding of local social service systems and resources, and knowledge of relevant legislation and regulations.

  Excellent communication skills, both verbal and written, with the ability to work effectively with diverse populations.

   Empathy, compassion, and strong interpersonal skills to build rapport and establish trusting relationships with clients.

  Strong organizational and time-management skills, with the ability to prioritize tasks and meet deadlines.

  Proficient in using computer applications and relevant software for documentation and reporting.

    Preferably a resident of SJDMC, Bulacan, Caloocan or nearby areas.
