Position: Licensed Practical Nurse (LPN) Care Coordination
Location: Alabang, Muntinlupa
Work Setup: Onsite, shifting schedule
Qualifications:- Active LPN or USRN license
- At least 6 months to 2 years of relevant healthcare experience
- Experience coordinating with multidisciplinary teams, including physicians, pharmacy, laboratory, radiology, and other healthcare departments
- Direct clinical experience in at least one of the following settings:
- Acute Care
- Ambulatory Care
- Care Coordination
- Home Health
- Strong communication, documentation, and organizational skills
- Ability to work in a fast-paced clinical environment
- Coordinate patient care activities across various healthcare departments
- Collaborate with healthcare providers to support continuity of care
- Assist in monitoring patient progress and care plans
- Ensure accurate and timely documentation of patient information
- Support patients and healthcare teams through effective communication and coordination
