Care Coordination - License Practical Nurse (USRN), Muntinlupa

Seeking USRN-licensed nurses with 6 months to 2 years of clinical experience to provide high-quality patient care in a fast-paced healthcare environment.

Qualifications:

• Active USRN license (required)
• At least 6 months to 2 years of bedside or direct clinical experience
• Strong clinical judgment and critical thinking skills
• Excellent verbal and written communication skills
• Ability to thrive in a fast-paced healthcare environment
• Applicants must be capable of working onsite at the designated location in Alabang, Muntinlupa City.
• Valid and updated NBI Clearance.

Key Responsibilities:

• Deliver safe, high-quality patient care in accordance with clinical standards.
• Assess, plan, implement, and evaluate individualized patient care plans.
• Administer medications and treatments accurately as prescribed by physicians.
• Coordinate and collaborate with doctors and multidisciplinary healthcare teams.
• Maintain complete, accurate, and timely patient documentation.
• Adhere to healthcare policies, protocols, and regulatory standards.

Schedule: Night Shift | Weekends off
