Patient Assessment:

• Conducting initial and ongoing physical and emotional assessments to identify patient needs and monitor their condition. 

Care Planning:

• Collaborating with physicians and other healthcare professionals to create and update personalized care plans. 

Medication Management:

• Administering medications, monitoring for side effects, and ensuring proper dosage and timing. 

Wound Care:

• Providing wound care, including cleaning, dressing, and monitoring for signs of infection. 

Patient Education:

• Educating patients and their families on disease management, medication instructions, and overall health maintenance. 

Vital Signs Monitoring:

• Regularly checking and documenting vital signs like temperature, pulse, and blood pressure. 

Personal Care Assistance:

• Assisting with daily living activities such as bathing, grooming, mobility, and eating, as needed.
